Text input and Submit Button

I am building a highscore table in my game. I need an input for the name of the player and submit button to add it to an attribute.

There is a lot of support for GUI widgets pack, which I have. But there is no documentation on how to actually use the pack. I can add the elements to a page, but how do I get them working.

Does anyone know of any tutorials or examples?
